GP Shared Care

GP Shared Care is an innovative model in which responsibility for a patient’s care is undertaken  collaboratively by a GP and another healthcare provider or providers.

GP Shared Care represents an innovative healthcare model wherein the collective responsibility for a patient’s care is collaboratively managed by a GP in conjunction with a multidisciplinary team of healthcare providers and clinicians. Shared Care Programs allow participants or patients to continue their care in the community while reducing pressure on our hospitals, outpatient departments and tertiary care systems.

At the heart of each program is our commitment to supporting GPs with the latest information, training and resources to enable them to continue to achieve the best possible health outcomes for their patients.

GPEx currently runs two shared care programs in South Australia:

We are now seeking to replicate this proven model in other geographical regions and to expand our into additional areas of clinical focus . Including but not limited to:
